Gingivectomy

Gingivectomy or Gum-Tissue Esthetic Recontouring (GER) is recommended when the gums are mis-shaped, over-grown from bacteria as a result of poor oral hygiene, or when the smile looks too “gummy”. Dr. Parajon performs Gum-Tissue Esthetic Recontouring using the Biolase® surgical soft-tissue laser after temporarily numbing the treatment area; there is no need for sutures afterward. Studies have shown that laser surgery provides the best postoperative healing.

 

PATIENT A

BEFORE: This patient had large gaps between every tooth before orthodontic treatment with Dr. Jorge Parajon. The large gaps meant long spans of gums between every tooth; after orthodontic treatment her gum tissue “bunched-up.

PROGRESS: This patient received GER therapy using Biolase® surgical laser, performed by Dr. Jorge Parajon, to reduce the size and dark color of the gums. Note the gum tissue is still undergoing primary healing at 1 week post-op.

AFTER: Weeks after primary healing of the upper arch, the tissue returns to its healthy, pink and tight condition. Gingival pigmentation is greatly reduced. Note that the post-surgical recontouring outline was preserved nicely. At this post-op appointment, the patient elected to have the lower arch treated as well with laser gingivectomy and recontouring. Above image shows the lower gum tissue 1 week after surgery.

 

PATIENT B

INITIAL CONDITION: Prior to starting comprehensive orthodontic treatment with braces.

FINAL ORTHODONTIC OUTCOME: After about 24 months of orthodontic treatment, the final tooth position is ideal. However, the gingival tissue is thick, irregular, hypertrophic and un-esthetic. Note that the teeth appear “square” due to the excessive coverage of the gums.

POST-SURGERY: Immediately after the gingivectomy across all the front teeth and back to the second premolars, notice the incredible improvement of the “golden” proportions of the teeth (height of the teeth over width).

FINAL: Post-surgical outcome after about 3 months of healing with excellent patient hygiene. The healing and stability of the outcome is amazing!

 

PATIENT C

BEFORE: Pre-orthodontic treatment condition. Note the irregular and asymmetric gingival outline, varying heights and bulbous tissue between the central incisors.

AFTER: Post-Orthodontics treatment using Invisalign aligners for approximately 12 months followed by Gingival Esthetic Recontouring of the 8 Upper front teeth. Note the improvement in the tissue symmetry, heights and contours (1-2 weeks post-op).

 

Soft Tissue Exposure

Soft Tissue Exposure is recommended when a tooth is under the gum tissue and must be uncovered. Dr. Parajon uses the Biolase® surgical soft-tissue laser after temporarily numbing the treatment area; there is no need for sutures afterward. This procedure is like a gingivectomy, except that it is generally performed in a different region of the mouth to make a tooth accessible for straightening purposes. This treatment mimics the natural process that occurs when of teeth grow into the mouth. Studies have shown that laser surgery provides the best postoperative healing.
